Case Management

Figuring out your health care after a hospital stay or a diagnosis can be scary. Our case managers are here to help in lots of ways. They can:

  • Provide motivation, tools, and lifestyle skills to minimize risk of complications
  • Help you understand your doctor’s instructions
  • Give you educational materials and free resources about managing your condition
  • Share resources available in your community
  • Teach you self-care techniques
  • Answer questions you might have about your medicines
  • Help connect you to resources to start planning for future healthcase decisions with advance directives

We offer Case Management to members who have chronic or complex conditions, like:

  • A high-risk pregnancy
  • A recent transplant
  • A major illness or accident
  • A high-risk infant or child
  • Behavioral health needs
